REQUIRED HHS ATHLETIC FORMS, COACHES FORMS AND EVENT FLYERS

This list of required forms and event flyers are available for your convenience. These forms and flyers will not be handed out by the coaches, so please review, as they contain important information critical to the players’ season. Please print all forms and return them to the coach (unless otherwise noted) by the posted due dates. Submitting forms earlier than noted due dates are encouraged and appreciated.

4) ATHLETIC PARTICIPATION FORM
(Which includes permission to participate and two emergency information sheets.)

6) (optional) Girls Speed Camp
